2.    PROOF OF CITIZENSHIP 

·         Certified State Birth Certificate  (Sent & returned w/Passport)

·         Naturalization Certificate

 

3.       PROOF OF IDENTITY

·         Driver’s License

·         Previous U.S. Passport

·         School Identification

·         Government Identification

 

4.       TWO (2) PASSPORT PHOTOGRAPHS

·         Identical 2” x 2” Color Photos (Taken within last 6 mos.)

                        (Nearest Location in Gallup, NM at: Wal-Mart / Walgreen’s / UPS Store)

 

5.     MINORS UNDER AGE 16 YEARS OF AGE

·         All minors must appear in person w/ Parent(s)

·         Both parents shall appear and sign application
